Originally a three-aisled basilica, the church saw the addition of two transept arms in the mid-14th century and an enlarged choir at the beginning of the 15th century. A significant event in 1479, known as the Miracle of Our Lady of St. John, attracted numerous pilgrims and led to the annual Procession of the Virgin Mary. Around 1500, the church was further expanded, and side choirs were added, complete with wooden barrel vaults.

Despite the iconoclastic fury of the late 16th century, which left many religious buildings in ruins, Sint-Janskerk sustained minimal damage. In the early 17th century, the original spire of the crossing tower was replaced with an octagonal dome topped by a lantern. The 19th century saw various restorations and changes, including the construction of a new sacristy in 1877. The 1930s brought the addition of a neo-Gothic baptistery, while post-World War I repairs uncovered late Gothic wall paintings beneath the 19th-century plaster.

The southern transept’s altar, also from 1670, is dedicated to Saint Sebastian.

The choir stalls, dating from 1776, are a fine example of Rococo craftsmanship. The paneling, from the third quarter of the 18th century, is designed in a classical style. The communion bench, pulpit, and four confessionals, all from 1780, are also in the Rococo style. In 1765, a rood screen and an organ, built by J.J. vander Haegen, were added. Although the organ was damaged during the French Revolutionary Wars, it was later repaired. A new organ, built by Jules Anneessens in 1921, was installed in the old casing and has since undergone several renovations, now boasting 19 stops on two manuals and a pedal.
